Roles and responsibilities

  • Provide one on one support, mentorship, and case management to post-majority clients.
  • Assist individuals in accessing housing, education, employment, healthcare, and financial supports.
  • Develop individualized support plans based on client needs, goals, and readiness.
  • Offer life-skills coaching such as budgeting, time management, household responsibilities and communication skills.
  • Maintain regular contact with clients through meetings, home visits, and check-ins.
  • Advocate on behalf of clients with service providers, community agencies, and government programs.
  • Work collaboratively with internal and external partners to ensure comprehensive wrap-around support.
  • Maintain accurate, timely documentation, case notes, and reporting according to policy.
  • Promote cultural connections and support clients in maintaining ties to community and identity
  • Respond to crises and provide emotional support, de-escalation, and referrals as needed.
  • Participate in team meetings, training sessions, and professional development.
  • Develop linkages and maintain effective working relationships with other community services/agencies, ensure data is updated on Path DMS, forms are properly filled out and filed, reports and other necessary documentation is accurate and submitted in the timelines mandated by the department.
  • Research information provided by SCN members to authenticate the need before taking action,  referrals that may include: mental health, addictions, suicide prevention, etc.
  • Conduct community and environmental assessments where feasible to identify and understand emerging issues, service gaps and opportunities to build capacity.

Skills:

  • Empathy and cultural sensitivity, providing advocacy and client engagement
  • excellent communication and writing, including report writing skills 
  • crisis management, trauma-informed practice
  • Computer efficiency in Microsoft, Google, Excel/Sheets, Outlook and PowerPoint.

Qualifications:

  • Diploma or degree in Social Work, Human Services, Child and Youth Care, or related field. 
  • Experience working with youth or young adults, preferably in a community or support setting.
  • Knowledge of local resources, post-majority programs and transitional supports.
  • Strong communication, interpersonal, and problem-solving skills. 
  • ability to work independently and collaboratively.
  • Valid drivers license and reliable vehicle. clear criminal record and vulnerable sector check.
