RESPONSIBILITIES:

• Perform comprehensive client intake and assess strengths, needs, and barriers

related to mental health and substance use issues.

• Develop individualized, client-centered case plans with recovery and treatment

goals. Maintain accurate and confidential client files and documentation.

• Ensure program and client approaches are evidence-based, using principles of

harm reduction and trauma-informed strategies.

• Ensure clients have access to supports, programs that will offer opportunities to

develop coping skills and build a support network for recovery.

• Offer emotional and practical support, and advocate for clients to access

necessary resources and services. Liaise and build relationships with internal

Friendship Centre programs and external community partners, including primary

care and social service providers.

• Participate in case conferences and team meetings to ensure integrated and

effective service delivery. Facilitate smooth transitions for clients between

services and into the community.

• Contribute to the development and evaluation of programs to promote mental

health and addiction awareness and prevention.

• Work to reduce health disparities among vulnerable populations, ensuring

services are culturally appropriate and relevant.

EDUCATION/EXPERIENCE:

• Diploma/Degree in Human Services, Social Work, or related field.

• Case management or community support experience required.

• Strong understanding of addiction, mental health, and social determinants of

health.

• Effective communication, advocacy, and organizational skills.
