Case Manager LPN Permanent Home Instead® is looking for a dynamic and caring person to join our team as a Case Manager LPN in Leduc for a permanent Monday to Friday position. An active LPN registration is required. The Case Manager LPN will work closely with families who are at the beginning of their care journey and are looking for care solutions for their loved ones, also supporting Care Givers, conducting training and holding family meetings.
